JOB DESCRIPTION

Registered Nurses are responsible for the assessing, planning, implementing, and evaluating individualized patient care delivery. The Registered Nurse providers clinical leadership to the patient care team for the assigned shift.

SUBMISSION REQUIREMENTS

• Shift Requirements: 8hr Days - 8:00 am - 5:00 pm
• Float Requirements: Home Health Hospice, will need a reliable vehicle
• On Call/Call Back Requirements: On Call Rotation For Call In's - On Call 5pm-8am (expected to call family back within 5 minutes and arrive to patients' home within 30-45 minutes) Call rotation is very limited and only based on call ins, or vacations all nurses are in the call rotation.
• Weekend/Holiday Requirements: 1 Holiday. 1 weekend every 6 weeks (if needed)
• Minimum Experience: 1-2 Years Hospice Experience. 1 travel assignment required.
• 1 Travel assignment required
• Charting Experience Required: Home Care Home Base (will train) – training modules can be completed prior to starting assignment in orientation
• Required Skillset: Med Surg, Hospice, Critical Care

CERTIFICATION REQUIREMENTS

• BLS (AHA)

• Indiana
• Compact allowed
