The physical therapist will accurately assessdifferentially diagnose and identify deficits in a patients abilityto move and perform functional activities in their daily lives.

They develop an individualized treatment plan with patientcenteredgoals to reduce pain restore function and prevent disability andwork with the individual to prevent the loss of mobility.

Essential Functions andResponsibilities :

• Performspatient evaluations through the process of obtaining historyperforming relevant systems reviews and selecting and administeringspecific tests and measures to obtain data.
• Establishes an individualized plan of care and selectstreatment techniques that address the specific impairmentsidentified in the evaluation and following physicianorders.
• Conducts ongoing reassessment of thepatient and their response to treatment.
• Initiates and manages the discharge plan beginning at thefirst visit and continuing throughout treatment.
• Supervises and delegates to ancillary staff and studentsconsistent with state laws and department policy.
• Completes legible accurate and clear documentation withinthe established timeframe identified in the department policy andprocedure and following accreditation requirements and insuranceregulations.
• Communicates with the patientfamily caregivers significant others and members of the health careteam to promote maximum benefit of care utilizing AIDET.
• Assesses educational needs of the patient familycaregiver or significant other and provides education to meet thoseneeds.
• Participates in professional growth anddevelopment through participation in educational programs includinginservice meetings and workshops and by reading current literatureconcerning the scope of practice.
• Activelyparticipates in quality improvement activities.
• Provides an environment conducive to safety for patientsvisitors and staff. Complies with appropriate and approved safetystandards.
• Functions within the scope ofpractice as defined by the states practice act.
• Performs other related duties as required anddirected.
