Mobile Care Chicago is Hiring a Mobile Asthma, Allergy and Pediatric Nurse (LPN/RN) Near Chicago, IL

About the Organization

Currently in its 24th year of service, Mobile Care Chicago is a non-profit organization which operates mobile medical clinics called Asthma Vans. These Vans travel into low-income communities and provide no-cost care, education, and treatment to children with asthma and allergies. Through two Asthma Vans, Mobile Care Chicago currently provides care to over 1,100 patients through 3,300 patient visits yearly.

About the Position

The Asthma Van Nurse (LPN or RN)’s role is to lead phone triage requests, patient intake, asthma education and inhaler and device technique evaluation, administer immunizations and/or injections (i.e., biologics such as omalizumab) and to perform allergy skin prick testing, and to assist the provider in the evaluation of patients in-person and remotely. The Asthma Van nurse will also lead triaging patient and pharmacy related requests including prior authorizations, medication renewals, sick/urgent calls and triage to the appropriate provider.

The primary responsibility of the nurse is to lead patient care efforts inside and outside the mobile setting to ensure continuity of care for patients and families and to triage for and support the medical providers’ treatment plan for families. This includes the following tasks:

Direct Clinic Care

  • Obtain history of present illness, complaint or symptoms, assess needs of the patient and appropriately triage to a provider and/or provide direct medical advice to the family.
  • Refill medications, as appropriate.
  • Complete Prior Authorization forms and provide supplemental information, request more from the provider on specifically what may be needed, when appropriate.
  • Call pharmacies and provider offices on the behalf of the provider under their direction.
  • Lead asthma education, device tutorials (including inhalers and epinephrine auto injectors) and help facilitate environmental trigger reduction education with patients and their caregivers.
  • Assist in precepting other staff to ensure that they are also meeting the standard of care at MCC both clinical and administratively, ensure adherence to proper inhaler technique and that individual(s) is up to date on asthma and other relevant treatment guidelines.
  • Administer immunizations and biologic medications.
  • Assist the provider in treatment of anaphylaxis and asthma exacerbations.
  • Perform allergy skin testing.
  • Complete necessary school and physical forms.
  • Perform spirometry and other breathing testing.
  • Obtain vital signs.
  • Maintain medical records in the Electronic Health Records (EHR) system, review and prepare EHR prior to patient exam, obtain missing documentation; document medical visits, observations, and related activities.

Clinic Support

  • Ensure medications on the Asthma Van are present, in-code, and proper dosages are available for different age/weight children and that dosing charts are visible in the Asthma Van.
  • Help arrange for durable medical equipment, such as nebulizer machines, when appropriate.
  • Arrange samples according to their drug class (i.e., inhaled steroid, antihistamine), to inventory our stock of samples and ensure they are in-code, to help facilitate donations of needed medications.
  • Complete administrative day duties and provide support to the clinical team during the administrative day.
  • Assist in the planning and implementation of MCC projects and grants related to clinical/administrative duties.
  • Support the timely relay communication to and from the Asthma Van and primary care offices.
  • Work with the school nurse and other involved school staff to ensure effective coordination of asthma/allergy care.
  • Respond to patient and pharmacy inquiries (both in person and by telephone) and relay information to the appropriate clinical staff based on the nature of the inquiry.
  • Follow-up on fax and lab requests and process requests for external clinical information both to/from Mobile Care Chicago.

Position requirements

  • At least two years of experience in a pediatrics or an allergy-immunology practice
  • RN or LPN with an active license in the state of Illinois without restrictions
  • CME is up to date and proof provided
  • Fluency in Spanish (speaking fluency with at least proficiency in reading and writing)
  • Medical Phone Triage Experience
  • Skill in communicating with persons from varying cultural and economic backgrounds.
  • Skill in taking and interpreting vital signs and in assessing for respiratory distress.
  • Meticulous organizational skills
  • Experience providing injections/immunizations
  • Excellent computer competency
  • Some participation in MCC events is required
  • Good sense of humor, and highly motivated
  • Most importantly, a passion for helping others
