Position Highlights:

As a valued member of our Hospice Care team, you'll receive patient assignments from our Hospice Operations Leader. Your primary goal will be to foster a positive work environment through effective communication while handling essential duties such as assessing, planning, implementing, and evaluating patient care. Collaborating with attending physicians, hospice staff, and other agencies will be essential for ensuring optimal care and resource utilization. Regular communication with our Hospice Operations Leader for care reviews and attending physicians will be crucial.
Your Responsibilities Will Include:
Maintaining thorough patient records to ensure clarity and accuracy in the care process. Exhibiting unwavering dedication to the hospice philosophy of care, supported by at least one year of recent acute care/hospice experience. Requirements:
