Overview

Missouri Baptist Medical Center, an acute care hospital in St. Louis County, offers a full continuum of medical and surgical services, including heart care, cancer, women and infants, breast health, gastrointestinal, orthopedic, stroke, therapy, wound and pain management services. The hospital has a 24-hour adult emergency department and cares for pediatric patients at a separate emergency department in collaboration with St. Louis Children’s Hospital.

The Care Coordination Department is comprised of RN Case Managers, Social Workers, Compliant Documentation Specialists and skilled administrative staff. Case Management in the Hospital setting is a collaborative practice model including patients, nurses, social workers, physicians, other practitioners, caregivers and the community. The Case Management process encompasses communication and facilitates care along the continuum through effective resource coordination. The goals of Case Management include the achievement of optimal health, access to care and appropriate utilization of resources, balanced with the patient's right to self-determination.

Preferred Qualifications

Role Purpose

Provides clinical social work services to address the psycho-social needs of complex patients and families. Provides individual and group counseling, linkage to community resources, expertise in cultural diversity and advocacy. Collaborates with the health care team and ensures timely, cost-effective discharge and transition planning.

Responsibilities

• Assess, prepare, and facilitate complex patients' personal, social, occupational, and emotional status to determine discharge needs. Facilitates Patient Care Conferences (PCC) to clarify care and discharge planning with treatment team. Provides creative patient discharge planning for unique situations. Assists with non-citizen patients for whom traditional resources are not available and/or are returning to their country of origin.
• Identify resources, patient needs and develop and implement on going care plans. Assists with implementation of new care coordination programs as assigned.
• Collaborates with community human service agencies to meet the needs of patients, families and the hospital. Provides managerial oversight of the inpatient involuntary psychiatric programs on the Pavilion and at PSC
• Serve as an expert witness by communicating experience to lawmakers, judges, lawyers and law enforcement. Manages process for Probate Court Petitions and hearings. Educates and prepares physicians to testify in the ECT hearings.
• Provides forensic consultation on patients who lack mental capacity, require treatment and/or discharge planning. Consult on Guardianship and provide necessary paperwork.
