Job Summary and Qualifications

Supports department by providing quality case management and discharge planning services for a specific group of patients hospitalized for treatment of psychiatric or emotional disorders, their families, spouses and significant others. Also, by serving as a member of the multi-disciplinary treatment team supporting the unit’s treatment program and philosophy, assuring the delivery of quality care.

What qualifications you will need:

• Required Education: Two years of college
• Preferred Education: Bachelors degree
• Required License: None
• Preferred License: None Required Experience: Two years of college AND minimum three years functional work experience in related field would be considered
• Preferred Experience: Three years of experience as stated above
