About the position: The Per Diem HHA will work in a hospice house environment and will be assigned responsibilities in a manner that promotes quality, continuity and safety of a patient’s care. The HHA is responsible for observing and providing patient care services to hospice patients.
Responsibilities:
· Performs personal care needs to hospice patients according to the care plan/supervisor, including: feeding, dressing, bathing, toileting, personal hygiene, changing bed linen, laundry, etc…
· Documents observations on patient’s condition, care performed and any significant changes
· Meets safety needs of patient and uses DME and other equipment safely and properly (hospital bed, side rails, O2, nebulizer, foot stools)
· Takes and records oral, rectal and axillary temperatures, pulse, respiration and blood pressure as appropriate
· Provides proper care and observation of patient’s skin to prevent breakdown
· Assists in ambulation, range of motion and exercises as instructed by the Supervisor
· Provides respite for patient’s/family/caregiver when on-site, as appropriate
· Keeps patient’s living area clean and orderly, as assigned
· Respectful of patient and family members and patient’s personal needs
· Documents HHA visits in EMR as per Mission House hospice standards and follows HHA care plan
· Attends monthly staff meetings and in-services
· Maintains required competencies and licensure to perform job
· Other duties as assigned
Qualifications:
· Current CA HHA license
· Current CPR certification
· One year experience as an HHA
· Computer proficiency in Microsoft Office and willingness to learn new software as needed
· Team player with excellent organizational, communication, and problem-solving skills
· Ability to read and document care that is given
· Ability to follow written and verbal instruction
· Demonstrates tact & patience
· Understands hospice philosophy & comfortable providing specialized care to the terminally ill
Physical abilities/requirements:
· Ability to safely lift/transfer 50 lbs with or without assisted devices (patients, supplies, etc)
· Ability to push, pivot, pull, and reposition 50 lbs with or without assisted devices
· Ability to bend, squat, stoop, crouch, kneel, reach, climb, and rotate
· Manual dexterity (grip/grasp/pinch)
· Ability to stand for prolonged periods of time
· Ability to walk for prolonged periods of time
· Ability to drive for prolonged periods of time
· Visual and hearing acuity with or without assisted devices
· Withstand different work environments
o Internal- office noise and interruptions, open workspaces
o External- a number of elements including but not limited to: various community & living environments, weather, & travel/traffic conditions
Please Note: The salary range provided here is only a reference guide, but final salary range depends upon work experience.
Pay: $26.00 - $30.00 per hour
