As a Case Manager with Sun Tree, you plan, organize and direct hospice care. If you're passionate about end-of-life care, we invite you to apply today.

Pay begins $34/hour and can go up depending on experience. We offer benefits, mileage reimbursement (around $200 a week for FT clinicians!), and minimal on- call requirement (We have a team for that!)

Currently, we are looking for a Registered Nurse Case Manager who would like to combine their love for people and excellent clinical and leadership skills with the opportunity to advance your career.

The Position

• Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness (es).
• Provides professional nursing care by utilizing all elements of nursing process.
• Assesses and evaluates patient’s status by writing and initiating plan of care, regularly re-evaluating patient and family/caregiver needs and participating in revising the plan of care as necessary
• Participates in on-call duties as defined by the on-call policy.
• Ensures that arrangements for equipment and other necessary items and services are available.
• Supervises ancillary personnel and delegates responsibilities when required.

• Graduate of an accredited school of nursing. 1 to 2 years of recent acute care experience and 1-2 years of recent experience in home care
• Good standing RN license in state of practice
• BLS Hands-On CPR certification.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
