Minnesota Merit System is Hiring a Public Health Nurse - Houston County Near Caledonia, MN

Houston County Department of Human Services is currently seeking applicants for a full-time (40 hours per week) vacancy for a Public Health Nurse in the Public Health and Human Services Department.
This position will work with various Public Health programs with primary focus on WIC, Maternal Child Health, Family Home Visits, and other outreach/screening programs involving families/children, and other duties as required. 
POSITION PURPOSE: A professional employee, acting under the general supervision of the Public Health Supervisor, a Public Health Nurse (PHN) is responsible for providing the full range of essential public health services enumerated in Minn. Stat. § 145A.02, Subd. 1a. Areas of Public Health Responsibility and Subd. 8a. Essential Public Health Services. Work in collaboration with Public Health unit colleagues to promote healthy communities and individual behaviors. 
For information on how to apply, see Examination Process section below. • Assesses client's needs and the level of service required and coordinates service providers.
• Provides direct skilled nursing service to clients as recommended by physician.
• Uncovers health problems through observation, interviews and analysis of records and initiates appropriate action.
• Teaches the client basic principles of healthful living applying information from related fields such as nutrition, dental hygiene, safety, etc.
• Teaches and supervises family members and auxiliary nursing personnel proper nursing and physical therapy care of the client as directed by physicians.
• Communicates/consults with the physician, nursing team, home health aides, social worker, and other involved caregivers to coordinate health care plans to meet client needs.
• Performs diagnostic tests as authorized and interprets the findings to client and/or families.
• Instructs caregivers to recognize early signs of disease and deviations from normal health patterns.
• Assists client in obtaining medical supplies and establishes appropriate client service fees.
• Maintains case file documentation.
• Provides school health services to assigned schools.
• Provides input in program planning, the development of agency goals and the evaluation of the agency's programs and policies.
• Participates on pre-admission screening teams, community child protection teams, etc.
• Encourages community development by initiating or participating in action to address specific community needs.
• Represents the Public Health Department by addressing various public and private organizations, community groups, and members of the general public concerning policies, programs, responsibilities and activities to inform and elicit community support.
• May be assigned to work in specific public health program areas, such as WIC, MCH, EPS, etc.
• Provide in-service education to personal care attendants, home health aides, and housekeeping personnel.
• May supervise and evaluate personal care attendants, home health aides, and housekeepers.Minnesota Merit System Minimum Qualifications - Public Health Nurse
Certification as a Public Health Nurse in Minnesota and current licensure as a registered nurse in Minnesota.

Bachelor’s of Science in Nursing from an accredited university/collegeCurrent licensure as a registered nurse in MinnesotaMN Public Health Nurse Certificate; met the coursework criteria set forth in MN Rule 6316.01000-OR-Registered Nurse degree from an accredited school of nursing or universityCurrent licensure as a registered nurse in Minnesota*Ability to obtain the MN Public Health Nurse Certificate within 1 year of employment-AND-Minnesota DHS Public Health Nurse Merit ratedOther Pre-Employment Standards/RequirementsPass a criminal history/background check The examination will consist entirely of a rating of education and experience.Note: This is a two step application process
