Minn Care Home Health provides home care services to adults with developmental and mental disabilities, and those in need of medical care after hospital discharge. Minn Care is looking for caring, fun and energetic Activities Coordinator to join our team! The DSP Activities Coordinator will provide direct care and assist program managers with activities for the residents we support.

MAIN RESPONSIBILITIES:

Develop a care plan with the client and responsible party based on the Assessment and Service Plan. The care plan must be completed within the first seven days of starting service with Minn Care Home Health.

Train staff on medication administration, maintain an accurate medication list, order medications for each client.

Train and orient staff on the individual needs of the clients.

Train staff based on the changes to the care plan and client’s needs.

Supervise and evaluate individual staff to ensure their competency to provide the required home care services.

Oversee the delivery of and evaluate the effectiveness of home care services in accordance with Minn Care Home Health and Department of Human Services policies:

Meet with each client receiving waiver services monthly

Meet with group home clients weekly

Communicate with clients’ medical care providers regarding clients’ conditions and medications.

Document the following: care plan, changes in client’s condition and/or medications, results from supervision visits, all communication with clients and staff

REQUIREMENTS

Registered nurse license in the state of Minnesota

Possess a great attitude and positive demeanor

Must pass the DHS criminal background check

Excellent communication and documentation skills
