Position Overview:
Mindful Wellness of AZ Integrated Clinic is seeking a compassionate and dedicated Psychiatric Nurse Practitioner to provide specialized mental health care to seniors and adults in the comfort of their own homes. As a mobile house call provider, you will have the unique opportunity to address the mental health needs of individuals who may be homebound or facing mobility challenges. Responsibilities:

• Conduct psychiatric assessments and evaluations for seniors and adults experiencing mental health concerns, including depression, anxiety, cognitive impairment, and other psychiatric disorders.
• Develop individualized treatment plans based on thorough assessments and in collaboration with patients, their families, and other healthcare providers.
• Provide medication management, psychotherapy, and other evidence-based interventions to address mental health needs.
• Educate patients and their families about mental health conditions, treatment options, and strategies for coping and improving overall well-being.
• Collaborate with interdisciplinary team members, including primary care providers, social workers, and other mental health professionals, to ensure comprehensive care coordination and holistic support for patients.
• Maintain accurate and timely documentation of patient encounters, treatment plans, and progress notes in electronic medical records (EMR) systems.
• Adhere to ethical, legal, and regulatory standards governing psychiatric practice and patient care.

Key Requirements:

• Current Psychiatric Nurse Practitioner license in the state of Arizona.
• Minimum of 2 years of clinical experience in psychiatric/mental health nursing, preferably with experience working with geriatric and adult populations.
• Strong assessment and diagnostic skills, with the ability to evaluate and address a wide range of mental health issues commonly experienced by seniors and adults.
• Excellent communication and interpersonal skills, with a compassionate and patient-centered approach to care.
• Ability to work independently and manage a flexible schedule, including traveling to patients' homes within the designated service area.
• Proficiency in using electronic medical records (EMR) systems for documentation and care coordination.
• Valid driver's license and reliable transportation.

Pay: $67.00 - $80.00 per hour
