MILWAUKEE CENTER FOR INDEPENDENCE INC is Hiring a CCBHC Intake Coordinator Near Milwaukee, WI

Job Purpose:

This is a professional position that is responsible for the care coordination of adults and children with mental illness. The intake coordinator will complete all initial documentation, screenings, and assessments as required by the Behavioral Health Outpatient Clinic and Certified Community Behavioral Health Clinic (CCBHC) regulations. This individual will also provide coordination of care as needed.

Essential Job Functions:

  • Conduct the intake of new clients to the behavioral health outpatient (BHO) clinic. This will include the tracking of the intake process and completing initial paperwork, screenings, and assessments for each new client in a timely manner and in accordance with, State, CCBHC, and Milwaukee County regulations.
  • Monitor and ensure that intake documentation meets all relevant local, state and federal standards, including DHS 34, 35, and 36 and Title 19 regulations.
  • Complete eligibility and payor verification for all new and existing BHO clients prior to upcoming appointments.
  • Maintain system of medical records management for all BHO clients in accordance with state and federal standards, including DHS 34, 35, and 36 and Title 19 regulations
  • Assist Front Office Manager with incoming referral process documentation and management as needed. This includes following up with incoming referrals as well.
  • Complete an initial phone screen with incoming referral clients covering topics such as: completing a brief questionnaire, verifying insurance/funding is documented in MCFI’s EHR, document client’s provider/ care team, and verify there is nothing that would prevent the client from working with WHCG therapists (e.g. being a family care client with no CCS funding).
  • Complete an initial appointment with clients before establishing care with a therapist. At this appointment the following information should be collected: complete consent
  • -to -treatment packet, complete screening tools (PHQ9, GAD7, Crisis Triage, SBIRT), collaborate with the Coordinator of Psychotherapy Services to determine which caseload new clients should be added to, and schedule initial appointment with the therapist.
  • Responsible for accurate documentation and maintaining case file as required by agency policy, government regulations and applicable local, state and federal codes.
  • Responsible for meeting department designated requirements, EHR note entry within 72 hours, and enter and update as needed client demographic information.
  • Attends and participates in CCBHC, BHO, and CFI staff meetings, in-service training seminars and conferences as required.
  • Responsible for adhering to agency policies and procedures while carrying out daily responsibilities.
  • Completing CCBHC National Outcomes Measures (NOMs) packets, and other CCBHC required documentation as needed.
  • Access to a personal vehicle, a valid Wisconsin driver’s license and insurance in conformity with agency limits.

Required Education, Experience, Certifications, Licensure and Credentials: (Where appropriate, education and/or experience may be substituted)

Minimum Required Education: Master’s Degreein the field of Social Work, Counseling, or related Human Services Field.

Minimum Required Experience: 1 year(s) of data entry experience preferred

Required License Certification Registration:

Issued by Wisconsin DSPS (if applicable)

Travel outside of office: 45%

Knowledge Skills Abilities:

  • Preferred Certification as a functional screener is desired but will provide training as needed.
  • Ability to clearly convey complex instructions and ideas both in writing and verbally.
  • Knowledge of using work related equipment and applications such as cell phone and texting, computers, photo copier, Microsoft Office, and Teams.
  • Knowledge with individuals with dual diagnoses is preferred.
  • Knowledge with motivational interviewing is preferred.

Physical Requirements, Visual Acuity, and Work Conditions:

Physical Requirements: Exerting 10-20 pounds of force occasionally. Reaching, extending hand(s) and arm(s) in any direction. Standing for sustained periods of time. Using Stairs. Walking, moving about on foot to accomplish tasks, pushing, pulling. Lifting, raising objects from a lower to a higher position or moving objects horizontally from position to-position. Picking, pinching, grasping items, typing, or otherwise working.

Talking, expressing, or exchanging ideas by means of the spoken and written word. Hearing, perceiving the nature of sounds at normal speaking levels with or without correction

Visual Acuity: The worker is required to have visual acuity to determine the accuracy, neatness, and thoroughness of the work assigned, preparing, and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading with or without adaptive equipment

Working Conditions: Work is performed largely in the community but may at times include an office environment or other various settings. The worker is subject to inside and outside environmental conditions, loud noises, and fluctuating temperatures and the occasional use of stairs.
