Job Overview:

We are seeking a dedicated Service Coordinator to join our team. The Service Coordinator will be responsible for overseeing and coordinating services for individuals with developmental disabilities, ensuring their needs are met effectively.

Responsibilities:

- Develop and implement service plans for individuals with developmental disabilities
- Coordinate services with various agencies and providers to ensure comprehensive care
- Conduct assessments to determine client needs and goals
- Monitor client progress and adjust service plans as necessary
- Provide counseling and support to clients and their families
- Collaborate with community resources to enhance client services

Skills:

- Strong administrative skills to maintain accurate records and documentation
- Excellent relationship management abilities to work effectively with clients, families, and service providers
- Knowledge of behavioral health principles and practices
- Experience working with individuals with developmental disabilities
- Background in social work or related field
- Budgeting skills to manage resources effectively
- Ability in program development and management
- Supervisory experience to lead a team effectively

Pay: $23.00 - $24.00 per hour

Experience:

• Social work: 1 year (Preferred)
• Case management: 1 year (Preferred)
