1. Develops, implements, administers, and evaluates family home visiting programs, including grant supported programs, in conjunction with government officials, health care providers, community resources, community committees, and the public based on the Zero to Three Home Visiting Community Planning Tool to meet community needs for family stability. Supervises staff case planning, establishes caseloads and schedules; reviews complex cases with staff and makes final determinations utilizing a reflective practice approach. Reviews case records to ensure that agency, state, and evidence-based family home visiting model policies, rules, and regulations are adhered to, and proper practices are being observed.
2. Uses independent judgment to provide supervision and leadership to unit personnel. Makes recommendations on matters of hiring and discipline; conducts ongoing feedback, coaching, mentoring, and reflective supervision, and timely formal reviews of performance of staff; provides orientation, training, and work direction, ensuring staff knows and follows department, County and evidence-based family home visiting model rules, as well as standards of nursing and sound work and safety practices, in order to accomplish job objectives and avoid injury or loss.
3. Serves as the Maternal, Infant, Early Childhood Home Visiting (MIECHV) Coordinator. This role includes, but is not limited to, administering an evidence-based family home visiting model; assuring County is meeting responsibilities outlined by the state contract and program model; developing and implementing a local continuous quality improvement process; collecting and reporting client benchmarks and constructs; evaluating program progress and outcomes; collaborating with a local mental health practitioner for reflective practice and case consultation.
4. Allocates human and fiscal resources, as authorized, to ensure unit goals are achieved in a cost-effective manner. Analyzes expenditures, expected needs, future cost projections, and sources of funding in order to develop detailed budgets; reviews year-to-date spending reports and monitors performance against plan during the entire budget cycle; works to ensure the most cost effective alternatives are identified when planning expenditures; prioritizes program elements based on community need; prepares requests for annual funding and presents to supervisor
5. Responsible for communications that maintain relationships key to the function of the County; Plans and carries out communications at a level that can impact the morale and performance of staff. Represents the agency at meetings and conferences with other agencies and/or individuals; meets with community groups or professional groups to explain the programs administered by the agency; fosters effective community-agency relations and organizes community projects. Handles escalated client issues; listens to the clients’ concerns and recommends appropriate action.
6. Develops and plans for ongoing program analysis and needs assessments to ensure requirements are met. Gathers information and prepares grants for family home visiting programs. Evaluates program outcomes and functioning of community partnerships. Makes modifications to family home visiting programs as needed. Provides reports, statistics, and information, including planning and community needs, to supervisor. Seeks grant funding and other funding sources for projects and unit purposes. Works with administration staff to account for grant fund uses. Prepares and supervises special project grant awards.
7. Works in conjunction with County Attorney’s Office in developing, coordinating, and maintaining contracts and memoranda of understanding for services, including insurance companies.
8. Analyzes, evaluates, and makes modifications in procedures; interprets federal, state, and local policies to supervisor, County Administrator, staff, and community. Recommends actions to be taken to the Board; implements decisions of the County Board, County Administrator, and Supervisor.
9. Provide direct skilled nursing service to clients.
10. Performs other job-related duties as directed.
