Midwest Hospice is Hiring a RN Case Manager Hospice Near Mason, OH

SUMMARY OF RESPONSIBILITY

The Registered Nurse provides end-of-life care to patients needing hospice services in their home or community setting. The professional nurse will provide case management, admission facilitation, clinical support, continuous care, after hours’ call, and/or other skilled nursing expertise with end-of-life care.

Essential Functions

1. Completes the initial, ongoing and comprehensive assessment of the patient and family to determine hospice needs. Assesses and evaluates patient’s status by: initiating plan of care, regularly re-evaluating patient and family/caregiver needs, and participating in revising the plan of care as necessary based on patient status and needs.

2. Counsels the patient and family/caregiver in meeting care needs of the terminally ill patient. Provides health care instructions to the patient and family/caregiver as appropriate per the assessment data and plan of care.

3. Documentation of patient difficulties, nursing assessments, patient goals, and care provided, and patient/family outcomes from interventions and care provided.

4. Initiates appropriate palliative nursing procedures. Makes recommendations for symptom management; administers medications and treatments as prescribed by the physician in the physician’s plan of care.

5. Assist the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency as consistent with the plan of care.

6. Provides and maintains a safe environment for the patient.

Communication

1. Initiates regular communication with attending physicians and hospice team members regarding the patient’s needs and reports changes in patient condition.

2. Communicates with community health related persons to coordinate the care plan.

3. Instructs the patient and family/caregiver of self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers.

4. Works in cooperation with the family/caregiver and interdisciplinary group to meet the emotional needs of the patient and family/caregiver.

5. Attends interdisciplinary group meetings.

Additional Duties

1. Participates in on-call duties as defined by the on-call policy.

2. Ensures that arrangements for equipment and other necessary items and services are available.

3. Monitors all documentation of clinical records to ensure compliance with regulatory standards of timeliness, accuracy, and completeness via electronic record so that problems, actions, and goals are accurately and clearly stated and changes are reflected as they occur, including new information obtained during IDG meetings.

4. Audits charts for clinical oversight and compliance and adherence to the Medicare guidelines.

5. Service is expected to be documented during, or by close of business day in order to maintain an accurate medical record.

6. Participates in all mandatory staff in-services, meetings, and/or education.

7. Fulfills the obligation of requested and/or accepted case assignments.

Job Type: Full-time

Pay: $70,628.87 - $90,972.40 per year
