Supportive Home Care Worker position available in Marshfield, WI
Up to 6 hours per week.
Days and times flexible.

SUPPORTIVE HOME CARE WORKER provides confidential services to physically or developmentally disabled individuals and/or frail elderly consumers in their home.
DUTIES: Supportive Home Care Worker will be trained in the provision of services and in each skill that is assigned. Completion of all time sheets and documentation necessary to maintain program services in a thorough, accurate, and timely manner. Maintains confidentiality of consumer's information. Adheres to and abides by all agency policies and procedures.
QUALIFICATIONS/REQUIREMENTS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.
Familiar with the philosophy and concepts of independent living required
Ability to understand and carry out consumer instructions required
Ability to relate well to others required
Good communication skills required
Good past employment references required
Effective means of transportation to the work site required. If consumer Plan of Care includes errand services or transport of consumer, a valid Wisconsin driver’s license, proof of auto liability insurance meeting agency minimum limits and ability to meet agency driving standards required.
Ability to: regularly sit, stand, and walk; use hands and fingers, handle, and feel; reach with hands and arms; bend; squat; twist and talk or hear; occasionally lift and/or move individuals using transferring techniques or assistive technology/equipment. Completion of interview and training conducted by PAS staff required
Satisfactory Criminal Background results required. Results based on relevance to position and in compliance with state and federal regulations for personal care workers.
