Job Title:
Phone Triage Nurse
Purpose Statement:
Keeping direct access for patients with their providers during busy business hours. The Phone Nurse will act as a provider liaison.
Supervision received:
Co-reporting responsibilities with providers and the practice administrators.
Supervision exercised:
May assign task to the floor nurse if the task is above the knowledge of the phone nurse.
Essential duties: (subject to modification)
1. Using nursing knowledge within the scope of the individual nurse to fulfill the duties.
2. Working in EHR in patient charts answering messages, filling medication refills, calling patient with test results that were reviewed by provider.
3. Triaging incoming phone calls from hospital, nursing homes, pharmacies, patients and their families.
4. Getting patients scheduled appointment rather than using the ER for non-emergent reasons.
5. Working with providers to help identify the need for Chronic Care Management.
6. Keeping communication between provider and patients with medical questions and advice given by provider.
7. Helping the front staff with questions regarding orders for labs or need for follow up appointment.
8. Helping keep cost down by changing medication to generic when able to.
9. Helping with prior authorization and medication refills.
10. Correlating medications with necessary lab test and the need for follow up appointments.
11. Risk stratifying patient by using the AAFP guidelines.
12. Attending all office meetings and group huddles.
13. Thoroughly documents and updates information in patient’s charts as required.
14. Initials and dates all entries to patient chart.
15. Reviews dates on medications on a regular basis and properly disposes of expired medications.

Current LPN or RN License

Requirements:
1. Occasional lifting of boxes of supplies, assisting patient in and out of the wheelchair, and on exam table. No lifting of patients in office.
2. Uses proper technique when assisting with patient transfers.
3. Manual dexterity sufficient to operate medical equipment, telephone, computer software and other office equipment.
4. Make patients feel important and that they are cared about.
5. Can identify nursing areas in the office that need improvement and can work on these with supervisor.
6. Must be precise and detail oriented, make few errors, and pay close attention to the work being performed.
7. Must be able to perform all work in a quick, accurate, and timely manner.
8. Find replacement nursing help when gone, outside of regular scheduled working hours, and indicates this on calendar.
Skills
1. Exercises, and uses, good judgment.
2. Can quickly, easily and accurately calculate dosages of medications.
3. Excellent phone skills including professionalism, courtesy, and accurate message taking.
4. Can operate medical equipment, computer, applicable section of practice management software, photocopy machine, and fax machine.
5. Can identify areas in nursing department that need improvement and can work on these with supervisors.
6. Quickly develops needed medical terminology and vocabulary.
Abilities:
1. Able to read, understand, and follow oral and written instructions.
2. Can give injections.
3. Able to accurately document and update patient information in medical record.
4. Able to establish and maintain effective and cooperative working relationships with employees, patients, and suppliers.
5. Is teachable and has a desire to continuously learn about the field of nursing.
6. Able to know when questions should be asked and does so to be sure things are done correctly.
7. Able to handle frequent stress from immediate demands and during peak periods.
