Middle Flint Community Service Board is Hiring a Case Manager, ADSS Program (Warner Robins, GA) Near Americus, GA

Position Title:  Case Manager, Addictive Disease Support Services

Description of Duties: (Warner Robins, GA)

Assist individuals with reducing barriers to employment; social and interpersonal skills; and improve family functioning that was affected by substance use through one-on-one services. Ensure linkages to all needed/required clinical services as requested by client, doctors, nurses, and stakeholders. Facilitate and/or coordinate meetings by ensuring all entities of client’s service delivery team are notified of client centered treatment meeting. Review w/each client monthly crisis recovery and/or safety plan. Ensure that the minimum contacts of 2 Face to Face or 1 Face to Face and 1 Collateral contact is made per client monthly. Complete all documentation within 24-48 hours from date of service delivery. Meet expected monthly and quarterly productivity standards as required. Ensure Failed Days for Failed Activities are three (3) days or less. Ensure Failed Days for Failed Claims are seven (7) days or less. Maintain a monthly CM unit authorization of 95% or better. Ensure there is no lapse in authorization of services. Complete daily/quarterly vehicle inspections and follow process for submission to Cooperate Compliance Dept. Schedule follow up appointments in a timely manner to ensure continuum of services. Attend staff meetings to discuss new and discharged cases to ensure that caseload is maintained at 1:50 ratio. Transporting individuals served is a requirement. 

Minimum Qualifications: High School Diploma or GED AND One year experience in a social services related position OR Two (2) years at an accredited college or university with at least 15 credit hours in a social services related field. Must have a valid, unrestricted Georgia Driver’s License. Must be keyboard proficient.

Preferred Qualifications:Experience working with adults with co-occurring Mental Health and Substance Use challenges and/or co-morbidities. Certified Peer Specialist - AD

Agency is an eligible work site for the Public Service Loan Forgiveness Program.

Minimum Entry Salary *: Based Upon Experience

Location: Houston, Peach, and Crawford Counties 

Services Area/Programs: WTRS IOP/Fresh Start
