MH Renal Services MSO LLC is Hiring a Mid Atlantic Practice Medical Director / MD, VA, NC, SC Near Baltimore, MD

Position Title: Mid Atlantic Practice Medical Director

Monogram Practice Medical Directors are quality-driven physicians who are passionate and dedicated to improving the well-being, quality of life, and health outcomes of our patients. We partner with the nation's leading clinical specialists to create transformative care. Monogram uses next-generation AI algorithms to predict necessary care to promote the delay of complex disease progression, such as CKD. Monogram is dedicated to creating a seamless transition to dialysis, pre-emptive kidney transplant, and conservative management. The practice medical director oversees the delivery of care management services within their market, ensuring high-quality patient care, by continually reviewing patient care plans and providing education and guidance to field clinicians when needed.

Responsibilities:

  • Oversight of large panels of health plan members in MD, VA, NC, and SC
  • Utilize IT and data tools to report, monitor, and improve quality and utilization
  • Conduct retrospective reviews of hospitalizations and dialysis treatments to ensure all services are effectively rendered
  • Oversight of implementation and refinement of clinical strategies, evidence-based guidelines, and care management programs for effectiveness and efficiency
  • Reduce unnecessary episodes of care including hospital readmissions
  • Prepare clinical care reports promptly
  • Lead case rounds with POD NPs and CMs
  • Analyze data to build member care plans and ensure care goals are achieved
  • Optimize patient health outcomes and total performance against health plan contracts
  • Represent Monogram as a leader to health plans, providers, and other organizations
  • Achieve clinical goals for delivering improved health outcomes at a lower cost of care

Requirements:

  • This is a hybrid telemedicine position with occasional in-home patient treatment visits and planning meetings requiring occasional domestic travel
  • Demonstrate experience applying evidence-based clinical criteria
  • 2 years of experience in CKD, ERSD, and chronic diseases
  • Strong management and communication skills
  • Experience with high-need Medicare Advantage and Medicaid population
  • Experience with NCQA, HEDIS, quality improvement, utilization management, and risk adjustment
  • Current medical license in MD, VA, NC, SC and willingness to become licensed in additional states
  • MD (medical doctor) from an accredited medical school
  • BC in an ACGME-approved specialty IM, Nephrology, CC, EM, or Palliative Care
