Shifts: Days – 8 hours; 40/week; Monday – Friday

Location: New York State; Hybrid (3 Field Days/ 2 Tele Days)

Pay Rate: $50 - $60 hourly

Duties and Responsibilities

We're looking for passionate Registered Nurses to join our client and assist individuals who need long-term home care in the communities we serve. The Nurse's main role will be to meet with individuals, usually in their homes or community spaces, and conduct a needs assessment to determine eligibility for government-sponsored assistance programs available for them based on their needs.

You will be part of an integral team that provides health assessments to our most vulnerable members of the community to ensure that individuals can receive the right kind of care.

Duties and Responsibilities:

• Outreach, schedule, and coordinate the assessment process with individuals, families, and providers in the home and/or community using a person-centered approach.
• Plan these meetings to fit into the individual's schedule, making it as convenient as possible for them.
• During these meetings, have open and person-focused conversations to really get to know what support the individual requires.
• Talk to other knowledgeable people in the individual's life and look at relevant records to make sure all the information we gather is accurate and complete.
• Coordinate with our support team to arrange for interpreters when needed, making sure everyone can communicate clearly.
• Use our software to fill out all the necessary assessment forms.
• Make sure your assessments are thorough and meet our quality standards, like completing everything on time and ensuring the individual is actively involved.
• Keep track of all your work and document everything according to our program's rules.
• Aim to meet or even exceed the number of assessments we expect you to do each month.
• Be ready to take on other tasks as assigned by your manager.

Requirements:

• A knack for organizing and coordinating schedules smoothly.
• Ability to communicate clearly and make people feel comfortable during assessments.
• Strong attention to detail and a commitment to accuracy.
• Strong Analytical skills and ability to use spreadsheet or database software.
• Comfort with using computer software for documentation.
• Ability to work one evening shift (5pm to 8pm) per week and ability to work two Saturdays per month

Education

• Bachelor’s Degree in Healthcare or equivalent

Experience

• 5 years of experience in Nursing, with at least 2 years in a Home Health setting

Licenses and Certifications

• Adult Basics Life Support
• Basic Life Support
• Active Registered Nurse License (NY)
