Metrocare is the largest provider of mental health services in North Texas, serving over 55,000 adults and children annually. For over 50 years, Metrocare has provided a broad array of services to people with mental health challenges and developmental disabilities. In addition to behavioral health care, Metrocare provides primary care centers for adults and children, services for veterans and their families, accessible pharmacies, housing, and supportive social services. Alongside clinical care, researchers and teachers from Metrocare’s Altshuler Center for Education & Research are advancing mental health beyond Dallas County while providing critical workforce to the state.

Coordinates follow-up, outreach, and psychosocial/rehabilitative services to persons with mental illness referred to Dallas Metrocare Services (DMS) Rapid Assessment and Prevention program. Services may include, but are not limited to, assessment, development and measurable treatment goals and objectives, referral, linkage, advocacy, monitoring, crisis intervention and continuity of care. The overall goal of this position is to successfully engage and/or re-engage consumers referred to DMS and maximize the potential level of functioning, reduce hospitalization, and aid in the successful reintegration of consumers into the community.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • To deliver services in compliance with regulations and standards of BHO, MCO, TXMHMR and the policies and procedures of Dallas Metrocare Services in order to facilitate the enhancement of individual consumer functioning in the least restrictive environment.

  • Meets established deadlines and performance expectations; promptly informs his/her supervisor of issues that may interfere with doing so
  • Follows written and verbal instructions, seeking timely clarification when needed.
  • Maintains the physical presentability of the work environment in a tidy and organized manner; reports facility maintenance needs to the local facility manager when observed.
  • Displays a positive attitude that promotes harmony and teamwork, and demonstrates an atmosphere of customer friendliness and professionalism.
  • Dresses in a professional manner, adhering to the agency dress code, and refrains from clothing that may be disruptive to consumers or other staff on the unit.
  • Maintains an acceptable work record by being punctual to arrive at work and return from breaks; requests Planned Leave within acceptable time frames established by the supervisor to insure adequate coverage is available on the unit
  • Attempts to perform all tasks with attention to a high degree of accuracy and professionalism.
  • Develops and maintains effective working relationships with co-workers and other inter/intra agency staff.
  • Follows the administrative chain of command in addressing work related issues.
  • Refrains from gossip or personal conversation within the hearing of consumers or collaterals.
  • Responds to requests for service in a professional and courteous manner.
  • Resolves conflicts in a manner that demonstrates respect for self and others.
  • Maintains current licensure status, as appropriate, and supplies Human Resources personnel with copies of current or renewed licenses/credentials prior to rendering/ billing for services.
  • Participates in continuing educational activities as required by appropriate licensing bodies, provides copies of CEU attendance if requested, and maintains current status on all agency required training.
  • Participates in regularly scheduled supervision, team staffings, and unit staff meetings.
  • Informs appropriate unit personnel of scheduled appointments or expected visitors to the unit. Refrains from bringing children and other family members to the unit except for brief periods of time on rare occasion.
  • Wears a Dallas Metrocare employee badge on the unit or at central administrative offices at all times.

  • Analytical skills, professional acumen, business ethics, thorough understanding of continuous improvement processes, problem solving, respect for confidentiality, and excellent communication skills.
  • Manage documentation associated with new referrals and existing consumers.
  • Provide overall service coordination, referral, linkage, and advocacy to consumers with varying needs. Perform duties in concert with other members of an interdisciplinary treatment team.
  • Coordinate incoming referrals and assignment to field-based clinicians.
  • Provide initial contact with consumers recently discharged from inpatient treatment.
  • Complete a psychosocial assessment on new admissions as needed. Formulate individual service plan based on assessment findings on new admissions as needed.
    • Develop measurable objectives and goals agreed upon by the consumer.
    • Actively assist in obtaining and modifying goals as needed.
    • Document attempts to involve consumer, service providers, and caregivers unless opposed by the consumer) in service plan process.
  • Review service plans every three months or as clinically indicated. 100% compliance expected.
  • Participate in interdisciplinary team staffing. Present psychosocial assessment findings. Encourage consumer to attend staffing. Update team on progress or lack of progress in reaching agreed upon goals. Provide relevant information that might effect treatment.
  • Follow-up assigned consumers who miss a scheduled clinic appointment. Document attempts to contact consumer. Report repeated unsuccessful efforts to contact the treatment team
  • Follow-up within 24 hours on assigned consumers who access emergency services. Report status-disposition to treatment team. 100% compliance expected.
  • Provide crisis intervention. Make home visits, initiate mental illness warrants, work with police and other public servants as a needed to address crisis.
  • Facilitate inpatient admission upon request of the treatment team. Provide continuity of services throughout inpatient stay. Participate actively in discharge planning. Contact new consumers within 2 days of discharge into the community
  • Provide individual, family, and group rehabilitation sessions. Evaluate progress of counseling session, solicit feedback from consumer(s), and consult with colleagues and team leaders when dealing with unfamiliar/uncomfortable issues

  • Complete computerized and/or written documentation of all services rendered. 65% direct service time is expected at all times
  • Maintain supportive documentation of all consumer related activities. Document in accordance to BHO, MCO, and agency standards and other contracting requirements. 100% compliance is expected
  • Maintain clinical record in compliance with all standards. Documents services in the automated record keeping system within 24 hours after service occurs
  • Screen applicants for service
  • Assist in triaging walk-in consumers, consumers in crisis, or scheduled consumers in the absence of the physician or other treatment team members

QUALIFICATIONS

EDUCATION AND EXPERIENCE:

  • Bachelors degree in Human Services with 1 to 3 years experience in MH/MR or related field or Masters degree with 0 to 2 years experience in Human Services.


    MATHEMATICAL SKILLS:
  • Basic math skills required.

REASONING ABILITY:

  • Ability to apply common sense understanding to carry out simple one or two-step instructions. Ability to deal with standardized situations with only occasional or no variables.

    COMPUTER SKILLS:
  • Use computer, printer, and software programs necessary to the position (i.e., Word, Excel, Outlook, and PowerPoint).
  • Ability to utilize Internet for resources.
    CERTIFICATIONS, LICENSES, REGISTRATIONS:
  • Current State of Texas Driver License or if you live in another state, must be currently licensed in that state. If licensed in another state, must obtain Texas Driver License within three (3) months of employment.
  • Liability insurance required if employee will operate personal vehicle on Center property or for Center business. Must be insurable by Centers liability carrier if employee operates a Center vehicle or drives personal car on Center business. Must have an acceptable driving record.
