Role Summary:

This position entails significant collaboration within an interdisciplinary team to deliver healthcare designed to meet the unique needs of each participant. Reporting to the Medical Director, the role focuses on participant assessment, care coordination, and primary healthcare delivery.

Essential Job Duties:

• Collaborate with the interdisciplinary team to develop and adjust comprehensive care plans.
• Perform initial, semi-annual, and unscheduled comprehensive assessments in person.
• Regularly visit participants in nursing homes to assess and maintain care continuity.
• Ensure effective coordination of care with community-based physicians and PACE physicians.
• Participate in discharge planning from both acute and long-term care settings.
• Analyze health outcomes and utilization data to drive improvements in practice and policy.
• Provide on-call coverage on a rotating basis during nights and weekends.

Job Requirements:

• Master’s Degree from an accredited School of Nursing.
• Certification as a Nurse Practitioner with Prescriptive Authority.
• Active Nurse Practitioner license in California.
• Minimum of two years’ experience with the senior population.
• Extensive knowledge of chronic care and geriatric issues.
• Valid State Driver’s License and a clean DMV record.
• Proof of current vehicle insurance is required.

Preferred Skills:

• Geriatrics experience.
