Responsibilities:

  • Work closely with therapists to coordinate services and support resident transitions within the facility
  • Ensure completion of intake process for new admissions, including assessments, treatment plans, and outcome measures
  • Coordinate with Utilization Review staff and program therapists to maintain authorization status and anticipate discharge needs
  • Develop master treatment plans for new admissions
  • Secure aftercare provider appointments within designated timeframes
  • Facilitate screenings for admission to various aftercare providers such as partial hospital programs, residential treatment centers, group homes, etc.

Qualifications:

  • Education: Master's degree in a mental health field from an accredited college or university
  • Experience: Four years of mental health or related field work preferred, with two years of direct practice experience
  • License/Certification: Current certification or licensure by the State of Texas (LMSW, LMFT-Associate, LMFT, or LPC Associate with required supervision)
  • Skills: Strong verbal and written communication skills, good clinical assessment skills, analytical ability, leadership abilities, flexibility
  • Essential Physical Requirements: Ability to perform medium physically demanding work requiring walking, sitting, or standing
