Mercy is Hiring a Hospitalist (Exempt) Near Chesterfield, MO

Mercy is currently seeking a board-certified Internal Medicine or Family Medicine physician with acute inpatient care experience to join our new and exciting Mercy Hospital-at-Home program in St. Louis. Missouri. As part of Mercy’s mission to transform the care experience, we are launching a new Mercy Hospital-at-Home (HaH) program, in partnership with Maribel Health, to provide acute-level hospital care to patients in the comfort of their homes.

Mercy and Maribel – Hospital at Home

Patients who have presented to the Emergency Department and have acute care needs that require hospitalization are thoroughly screened and admitted in the Mercy HaH Program. Once “admitted”, these patients are transported home to receive all the acute care services during the typical 3–5-day admission. This includes twice daily nursing visits, daily physician visit, 24/7 medical direction oversight, medication management, equipment, and all additional program support during HaH episode of care.

Hospital-at-Home Physician

The HaH Physician is a physician leader with strong acute care experience and comfort with providing high-quality patient-centered care to acutely ill patients at home. In this role, the physician will provide daily acute care to patients on the HaH services in both a virtual and in-person capacity and oversee the medical direction and overall care of the patients with support from the HaH interdisciplinary team. The HaH Physician will provide continuity between care settings, ensure home care and acute care protocols are seamlessly integrated in the home settings, and function as a lead clinician in the home environment.

This Position Offers:

  • Provide clinical care to acutely ill patients on HaH service by providing either virtual or in person visits daily depending on patient needs and circumstances.
  • Provide acute-level care as a key member of the Mercy HaH team including clinical documentation, treatment plans, medication management, and medical direction oversight.
  • Provide medical direction oversight to HaH clinical field staff and interdisciplinary team to provide high-quality clinical care and outcomes.
  • Provide direct supervision and oversight as a Collaborating Physician for dedicated Mercy HaH APPs.
  • Perform a daily patient care visit and all required clinical documentation, orders, chart review to meet the HaH policies and procedures.
  • A multidisciplinary care team to help identify patients that meet HaH criteria and provide clinical care to acutely ill patients
  • Facilitate in-home diagnostics, point-of-care testing, administration of medications and other treatments, patient monitoring, and any other patient needs as they arise
  • Participate with quality measurement and/or quality improvement initiatives to advance the work of HaH and other alternative care pathways, including advancements or innovations of the care model
