POSITION SUMMARY:
The Clinical Supervisor is responsible for the delivery of client care services through coordination, implementation, and supervision of the client's plan of care and supervision of staff delivering care. The Clinical Supervisor shares responsibilities for the agency’s overall compliance with State and Federal guidelines and standards of private duty nursing practice.

DUTIES AND RESPONSIBILITIES:
Intake and coordination of a referral form with referral source
Initial in-home health assessments and evaluations
Establish clients’ individualized treatment and medical record baseline
Collaborate with physicians, other agency staff, and contract services to coordinate and implement the POC
Supervises implementation of the POC and the quality of care delivered to the client
Provides in-home supervision of nurses and home health aides by directing, implementing, and evaluating the quality and appropriateness of care and following of the POC/service plan
Re-evaluates the POC at least every 60 days, when there is a change of condition, and after a hospital stay, revising the POC as needed to achieve client goals
Documents direct client services, coordination and collaboration with physician and other disciplines or providers in the care of the client, ensuring that documentation is complete, and complies with acceptable homecare standards and agency policy
Facilitates active and effective communication with team members as demonstrated through leadership of client case conferences, staff education and in-services, and timely clinical decisions which provide guidance in the delivery of client care
