CLINICAL SUPERVISOR

REPORTS TO: DIRECTOR OF OPERATIONS

POSITION SUMMARY:

The Clinical Supervisor is responsible for the delivery of client care services through coordination, implementation, and supervision of the client's plan of care and supervision of staff delivering care. The Clinical Supervisor shares responsibilities for the agency’s overall compliance with State and Federal guidelines and standards of private duty nursing practice.

QUALIFICATIONS:

  • Must be a Registered Nurse with a current, clear license in the state of Michigan
  • Must have at least one year of clinical experience obtained within the last 36 months

PHYSICAL QUALIFICATIONS:

  • Must be able to communicate effectively in English
  • Must be self-directed
  • Must be able to spend time in the office and in the field
  • Must be able to travel to client’s places of residence
  • Must be able to lift and transfer clients using proper body mechanics
  • Must be able to use computerized electronic records system

DUTIES AND RESPONSIBILITIES:

  • Supervise and direct all clinical services
  • Demonstrate leadership and provide direction to members of the client care team through case management.
  • Case Management of client services include:

Intake and coordination of a referral form with referral source

Initial in-home health assessments and evaluations

Establish clients’ individualized treatment and medical record baseline

Collaborate with physicians, other agency staff, and contract services to coordinate and implement the POC

Supervises implementation of the POC and the quality of care delivered to the client

Provides in-home supervision of nurses and home health aides by directing, implementing, and evaluating the quality and appropriateness of care and following of the POC/service plan

Re-evaluates the POC at least every 60 days, when there is a change of condition, and after a hospital stay, revising the POC as needed to achieve client goals

Documents direct client services, coordination and collaboration with physician and other disciplines or providers in the care of the client, ensuring that documentation is complete, and complies with acceptable homecare standards and agency policy

Facilitates active and effective communication with team members as demonstrated through leadership of client case conferences, staff education and in-services, and timely clinical decisions which provide guidance in the delivery of client care

  • Build strong rapport with client, their family as appropriate, by ensuring consistent open communication
  • Project a concerned, professional attitude and build strong relationships with staff, case managers, referral sources and supporting disciplines
  • Review and act to resolve complaints made by clients/families regarding care provided
  • Review and act to resolve complaints made by staff regarding policies and procedures
  • Assist Director of Operations and Care Coordinators to ensure staffing needs are met
  • Ensure adequate orientation and ongoing education of nursing and home health aide staff
  • Coordinate and ensure that the client is well serviced through the assignment of skilled staff and personal caregivers whose skills and abilities meet the clients needs
  • Provides timely documentation and meets professional standards of practice, establishes compliance with State and Federal regulations and ACHC standards
  • Reviews charts to assure compliance, assesses client’s progress, evaluates client’s needs for re-certification, and evaluates the effectiveness of client care delivered and achievement of established goals
  • Participates actively in Quality Improvement activities
  • Provides clients and staff supervision 24 hours a day when assigned on-call
  • Assists in the performance evaluations of staff members
  • Coordinate with billing department and insurance programs to ensure client service authorizations are accurate and up to date
  • Other duties as assigned
