Mercy Community Healthcare is excited to announce that we are hiring primary care providers at our Franklin, TN location. Applicants must have M.D., D.O., APRN, or PA licensure. Primary care providers will be responsible for maintaining the high quality of medical care provided to all patients of Mercy. As a Federally Qualified Healthcare Center (FQHC), Mercy primarily serves uninsured and underinsured patients in our community.

Job Description:

• Responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination for patients and families with acute and chronic health needs commonly encountered in general primary care practice for pediatrics and adults.
• Functions independently to perform age-appropriate history and physical for complex acute, critical, and chronically ill patients, which may include psychosocial, cognitive and/or personal.
• Collaborating with multidisciplinary team members by making appropriate referrals.
• Facilitating staff, patient and family decision making by providing educational tools, monitoring patients/family response to education and acts as a resource in implementing education programs to improve management of patients/families.
• Participating in Patient Centered Medical Home (PCMH) Care Team Collaboration.
• Carry a traditional patient panel and maintain the ability to see same-day and walk-in patients as needed.
• Perform traditional office-based procedures.

Requirements:

• Graduate of an accredited Medical or Osteopathic Medical School OR Graduate of an accredited school of nursing and Graduate of an accredited Nurse Practitioner Program or Physician Assistant Program.
• Unrestricted licensure.
• 2 years' experience preferred.
• Basic Life Support (BLS).
• Spanish speaking a plus but not required.
• Ability to work full-time hours (4 days per week).
• Experience with eCW EMR preferred.
