Job Description: The Corporate Director of Case Management is a key leadership position responsible for the assessment, planning, coordination, development, and direction of Case Management/Transitional Care, Post-Acute Services support, Social Work programs, and Case Management-related Regulatory Standards. This role extends to initiatives in areas such as automation and information systems. The Director will collaborate with cross-functional teams to implement comprehensive programs, identify improvement opportunities, and standardize processes to enhance outcomes and organizational efficiency. Expected Job Duties:
Ongoing evaluation and assessment of facility practices and industry changes related to Case Management/Transitional Care, Patient Choice, Referral Process, Avoidable Days, ED Case Management, and Social Work programs.
Conduct needs analyses, identify gaps, and develop program components to mitigate identified gaps.
Develop and implement evidence-based toolkits to support Case Management initiatives.
Establish, implement, and evaluate the strategic plan for clinical aspects of case management.
Work with Case Management Analytics to create data priorities and report formats displaying critical outcome, process, and operational factors.
Ensure case management and discharge planning policies comply with governmental and regulatory agencies.
Provide feedback to Case Management Leadership and other clinical areas using the department-approved process for data reporting.
Provide input for resources needed to deliver Case Management strategies effectively.
Develop and maintain effective relationships with related departments to promote Case Management goals.
Design, develop, implement, evaluate, and revise Case Management programs for optimum patient outcomes.
Collaborate with the Corporate Case Management team to develop policies and procedures supporting systems, standards of practice, regulatory standards, and evidence-based best practices.
Develop Care Coordination education and training in collaboration with the Director of Case Management Education.
Direct and develop Discharge Planning tools to support Case Management initiatives.
Responsible for integrating clinical documentation and electronic health record technology into the program workflow, ensuring technology supports Clinical Case Management.
Education/Experience Requirements:
Bachelor's degree in Nursing required or licensed social worker (LCSW).
Master's degree preferred.
5 years of Case Management experience required.
3 years of Case Management leadership experience required.
