Job Summary:

We are seeking an enthusiastic Outreach Specialist to join our team. The ideal candidate will be responsible for working primarily with older adults who are categorized as at-risk and need additional assistance to maintain their independence.

Duties:

-Assess the unmet needs of older individuals and encourage them to use existing services programs and benefits.

-Complete an individual-needs assessment for clients when service needs are requested; follow up on referrals from doctors, hospitals, and other agencies; visit the homes of potential clients to assess for chore, homemaker, and transportation.

-Provide education and consultation of MCCoA services and activities; link clients to available services and programs by direct help or by referral.

-Hand out materials such as brochures and newsletters to clients when they come into the office or if visiting them in their homes; assist with insurance questions – Medicare, Part D prescription drug coverage and other health care-related questions; participate in health fairs or other community-based gatherings to disseminate information.

-Listen to clients’ needs and assist them with phone calls, complete paperwork or provide information that will direct them to an agency that can help; teach classes that improve quality of life such as Matter of Balance, Tools for Caregivers, and Healthy Living with Chronic Conditions; identify topics of interest to the senior population for possible seminars.

-Computer operation; use of modern office equipment.

-Administrative abilities to handle documentation and reporting tasks.

-Ability to carry out instructions in written, oral, or picture form; deal with problems involving several variables within familiar context; recognize unusual or threatening conditions and take appropriate action; define problems, collect data, establish facts, and draw valid conclusions; exercise independent judgment and discretion; understand, interpret, and apply rules or regulations to specific situations; communicate effectively with seniors and develop sensitivity to their unique needs; work with clients with physical, mental, social, and intellectual disabilities; maintain confidentiality; remain calm; recognize safety warnings; work alone on most tasks; handle sensitive inquiries from and contacts with officials and the general public; develop and maintain effective working relationships; continue education through webinars, seminars, and classes pertinent to the needs of clientele; stand, sit or remain in a stationary position for a long period of time; ascend/descend stairs to enter/leave clients’ homes during visit; travel to and gain access to work sites.

- Knowledge or experience in public health initiatives is a plus
