Earn $38.93/hour.

Position Details:

As a vital member of our ACT Team, the Registered Nurse will collaborate closely with psychiatrists, mental health professionals, and various specialists to deliver comprehensive services to adults with serious mental illness. This community-based role involves providing therapy, medication management, and resource linkage through home visits and community engagement. The RN will serve as the primary point of contact for consumers' physical health needs and work in tandem with the multidisciplinary team to ensure holistic care.

• Manage medication in compliance with Merakey Medication policies.
• Provide comprehensive medication education to consumers.
• Administer Long Acting Injectable (LAI) medication as prescribed.
• Assist consumers in identifying and accessing necessary medical services.
• Coordinate service delivery by internal and external team members related to medical services.
• Conduct initial nursing evaluations and yearly updates for each consumer.
• Monitor symptoms and side effects of medication, providing education and documentation.
• Maintain pharmaceutical and treatment supplies for assessment and treatment purposes.
• Engage consumers in healthy lifestyle education and promote wellness behaviors.
• Act as a liaison for consumers with external medical providers and natural supports.
• Educate consumers' natural supports on diagnosis and mental health treatment.
• Provide cross-training to the ACT Team in medication education, physical health needs, and smoking cessation.

The ideal candidate will possess the following qualifications:

• Active Registered Nursing license
• Valid PA driver's license
• 2 years of clinical experience working with individuals with mental illness preferred
• Knowledge and understanding of recovery principles and working in a recovery-driven system
