We are seeking a Care Navigator to join our team at our program in Philadelphia, PA.

Earn $45,900/annually

The Care Navigator is an integral part of the medical and clinical teams. They assist with care management and program development at two Merakey outpatient substance use treatment sites. They build strong relationships with clients to help them reduce barriers to achieving their health outcomes. This includes conducting risk assessments, health education, and linkage and retention for patients receiving medication-assisted treatment, hepatitis, HIV, STIs, and PrEP/PEP services.

Position Details

  • Perform risk assessments, risk reduction education, and outreach with all individuals receiving integrated health services.
  • Collaborate with medical providers and nursing staff to communicate test results and report positive test results to the Philadelphia Department of Public Health.
  • Secure prior authorization for medications, ensure timely delivery of patient's medication, and complete successful medical service referrals.
  • Document and track patients' testing and treatment for Hepatitis, HIV, and other STIs and all patient interactions using the electronic medical record and Microsoft Excel.
  • Promote and coordinate PrEP/PEP and sexual wellness counseling and education for clients.
  • Attend case conferences, trainings, and program meetings.
  • Demonstrate respect and regard for the dignity of all patients and fellow employees to ensure a professional and responsible work environment.

Requirements

The ideal candidate will possess the following qualifications:

  • High School Diploma is required/Bachelor's Degree is preferred.
  • At least one (1) year of experience working in case/care management and/or treatment navigation.
  • At least one (1) year of experience working in Hep C/HIV prevention, PrEP/PEP treatment, and treatment of other STIs; and individuals with socially, culturally, and economically diverse populations.
  • Proficiency in Microsoft Office (Word, Excel, Outlook, PowerPoint).
  • Able to work both independently and in a team-based care environment.
  • Previous experience using an Electronic Medical Record (EMR) is preferred.
  • Bilingual (English/Spanish) is preferred.
