MHS is looking for experienced Home Based Case Managers (CPST) to serve Clark and Madison Counties. Case Managers work FT, Monday-Friday helping clients in the Springfield, London and surrounding areas. Hours may vary based on client needs and schedule flexibility. Case Managers (CPST) facilitate the delivery of services to clients in need in the community, and may assist with independent skills training, advocacy for financial, medical and legal entitlements. and arranges for crisis interventions when needed. They facilitate the rehabilitation process for clients and families; monitoring and will coordinate the delivery of necessary evaluations, assessments to identify the patient needs, development of the rehabilitation plan with patient and significant others, linkage and coordination of appropriate community services. .
