Mental Health Association of Oklahoma is Hiring a Street Outreach and Crisis Response Case Manager Near Oklahoma, OK

The Street Outreach & Crisis Response Case Manager will provide support to the OKC Fire and Police Department, City of OKC and the Street Outreach Crisis Response Team Lead. This position’s target focus are individuals who are identified as the top utilizers of city emergency services. Direct services provided to referred individuals may include assessing needs, identifying gaps, advocating, and facilitating warm hand offs to needed services like mental health, healthcare, housing, recovery services, socialization, and legal assistance. 

PRINCIPAL DUTIES AND RESPONSIBILITIES:

  • Review and evaluate referrals from the OKC Fire and Police Department, City of OKC and the Street Outreach Crisis Response Team Lead.
  • Participate in outreach at the direction of the Street Outreach Crisis Response Team Lead, in order to build rapport with clients, sign appropriate releases of information, and access needs. 
  • Identify gaps for clients and work with Street Outreach Crisis Response Team Lead to develop goals and objectives for individuals according to participants needs and preferences. 
  • Provide ongoing support, monitoring, and coordination of services for each participant. This includes regular face-to-face meetings and/or phone calls with participants or on behalf of participants.
  • Complete necessary documentation and data collection per program requirements.
  • Work within established data systems to evaluate participant improvement over time and monitor success of referral actions. Contribute to data entry, assessment, and reporting to support ongoing program development and community accountability.
  • Performs other duties as assigned. 

KNOWLEDGE, SKILLS & ABILITIES:

  • Bachelor's degree in social service/human services preferred or a relevant combination of education and experience.
  • Excellent organization and time management skills.
  • Must possess working knowledge of technology of both Microsoft and Google suite programs.
  • Ability to communicate clearly and effectively, both verbally and in writing.
  • Ability to work closely with a multidisciplinary team that includes various partner agencies.
  • Possess clean MVR and valid Oklahoma Driver’s License. 

WORK CONDITIONS & PHYSICAL DEMANDS:

  • Work is performed in both a climate-controlled office setting, as well as outreaching to encampments and other urban outdoor environments.
    • 70% of this position is performed driving/riding in a vehicle around the Oklahoma City Metropolitan area.
    • 20% of this position is performed outside working in various weather conditions (extreme heat/cold).
    • 10% of this position is performed in a climate-controlled office setting.
  • Noise level is moderate and includes sounds of normal office equipment (printers, fax machine, phones, etc.) as well as encountering outside noises like cars, freeway sounds, etc.
  • Required to work in area inhabited by unsheltered persons, to include working in dirty environments and dealing with difficult or challenging individuals. 
  • Potential exposure to stressful and/or physically challenging situations that could result in risk to one’s safety (unleashed dogs, aggressive people, etc.)
  • Work requires a flexible work schedule (ranging from 7:00am - 9:00pm) due to crisis hotline calls, internal, and external meetings. Occasional travel will be required, must have a valid Oklahoma Driver’s license. 
  • Ability to effectively communicate orally and in writing.
  • Physical ability to bend, stoop, climb, and lift 25 pounds on regular occasion.
  • Physical ability to partner lift and carry 50 pounds. (ice chest, food supplies, totes, etc.)
  • Visual and auditory ability to identify and response to environmental and other hazards of the site and staff behavior.
  • Physical mobility and endurance to perform tasks while standing/walking for long periods of time (60 minutes or more).
  • Ability to safely and properly use office equipment.
