MEMPHIS HEALTH CENTER is Hiring a Clinical Services Associate Near Memphis, TN

POSITION SUMMARY:

The Clinical Services Associate is responsible for conducting regularly scheduled audits to evaluate the company's performance in quality improvement and working with multidisciplinary teams to identify strategies and activities that lead to measurable improvement in health care services and health outcomes of individuals and populations.

ESSENTIAL DUTIES/RESPONSIBILITIES:

Assists the Clinical Services Director with HRSA clinical quality measures, Uniform Data System (UDS), Healthcare Effectiveness Data and Information Set (HEDIS), and other reporting programs.

Monitors performance improvement initiatives and supports compliance with HRSA Health Center Program requirements and Federal Tort Claims Act Program requirements.

Work collaboratively with the department managers to ensure that all patient’s complaints/grievances are managed according to established protocol and opportunities identified to improve patients’ experiences.

Coordinates and conducts walk-through audits and Environment of Care Surveys to determine compliance with accrediting and regulatory organizations standards.

Updates the balanced scorecard with quality and financial performance indicators monthly for reporting purposes.

Conducts semi-annual patient experience surveys, summarizes results, and reports findings to CQI/RM committee.

Assists with the management of the computerized data repository, including generating population health data reports, utilizing data for population health management, and addressing gaps in service and care.

Participates in state-level PCMH training sessions to gain a better understanding of the latest recognition requirement structure: concepts, competencies/core and elective criteria.

Coordinates training activities related to PCMH data collection, patient tracking, policy and procedure development, and workflow changes to ensure continuous compliance with recognition requirements.

Oversee the activities of the PCMH Coordinator and collaborates to develop strategies to address and close gaps in care.

Assists with the collection, integration, and presentation of diverse data related to clinical/operational performance and patient-centered medical home activities for Quality Performance Assessment Support System (Q-PASS).

Maintains a practical knowledge of tools and techniques of continuous quality improvement, including analysis and interpretation of data using computer-based disease registries (e.g., Athena, i2i, etc.) data collection systems.

Responsible for Infection Control Protocols and internal audits across different locations to ensure that we are operating according to guidelines.

Oversee and be able to be a point of contact/leader when Director of Clinical Services is out of the office for all covered departments under the umbrella of the Director of Clinical Services.

EDUCATION/EXPERIENCE:

Licensed to practice as a registered nurse in Tennessee.

Experience in working with a quality improvement or patient-centered medical home program.

Minimum 2-3 years of experience in a healthcare facility, preferably with a federally qualified health center.

Must demonstrate integrity, sound judgment, demonstrated leadership skills, and strong interpersonal skills.

Must be able to approach staff about quality issues with tact and diplomacy.

Experience working with medically underserved and indigent populations helpful and knowledge of health disparities highly desired.

Excellent oral and written communication skills needed and strong organizational ability required.
