Memory Matters is Hiring an Activities Coordinator Near Hilton Head Island, SC

Memory Matters is seeking an experienced and dynamic Activities Coordinator. We are a non-profit organization whose mission is to provide compassionate memory care programs, support for caregivers, and brain health education to the community. Position Summary: The Activities Coordinator is responsible for the recreational programming at Memory Matters Adult Day Service Program and ensures the delivery of high-quality programming to enhance the quality of life for both caregivers and participants. The Activities Coordinator is responsible for developing and implementing adult educational and recreational activities, for in-person programming in the Hilton Head Island and Bluffton locations. Programs are designed to meet the needs of individuals across the full brain health spectrum including those interested in maintaining a healthy brain, those with early memory loss and those with various forms of dementia. The Activities Coordinator works in collaboration with other program team staff and volunteers. The position is full time, non-exempt, and reports directly to the Program Director.
Job Responsibilities:
  • Responsible for the daily operation of the assigned programs including the social model adult day programs. Duties include, but are not limited to, implementation of curriculum by leading group activities, both virtually and on-site at Memory Matters, supervision of volunteers, safety of participants, attendance records, and client files.
  • Completes timely and accurate participant notes, charts and other documentation related to both program participants and family care partners, including data entry following HIPPA guidelines and SC DHEC regulations.
  • Assures that medical histories, medical directives, emergency contact information and emergency response plans are in place.
  • Works collaboratively with program team staff and volunteers in all programs. 
  • Foster’s peer support and coordinate information and care requirements with other direct program staff.
  • Maintains professional and technical knowledge about cognitive impairment.
  • Participates in regular staff meetings, staff training programs, and team coordination sessions.
  • Maintains client confidentiality per Memory Matters policy and HIPPA/DHEC regulations.
  • Serves as Memory Matters ambassador in the community.
  • Participates (as requested and time availability permitting) in Memory Matters special events and educational presentations.
  • Other duties as assigned by the Program Director.
Job Requirements:
  • Bachelor’s degree in education, health promotion, social work, human services, psychology, music therapy, art therapy, gerontology, or a minimum of two years of experience facilitating recreation group programs, comprised of diverse individuals with complex needs.
  • Experience working with the elderly, those living with Alzheimer’s disease and other dementias preferred
  • Experience with cognitive impairment and teaching preferred.
  • Experience with volunteer management a plus.
  • Ability to work independently to develop curriculum for the program
  • Knowledge of case management systems and relevant supplemental resources.
  • Excellent oral and written communication skills and problem-solving skills.
  • Experience working in an adult day services program or long-term care setting.
  • Proficient computer skills.
  • Must serve and protect Memory Matters by adhering to agency policies, procedures, professional standards, as well as federal, state, and local requirements.
