Melwood is Hiring a FAMILY SUPPORT ASSISTANT /Must Have Insured Reliable Transportation - NANJEYMOY, Near Upper Marlboro, MD

Description

Provide and/or assist with personal care and emotional support for people supported; assist the individuals towards reaching maximum independence based on their Person-Centered Plan. Engage persons-served in meaningful community/recreational activities.

Maintain the confidentiality of information for each person supported.

Communicate with the Program Manager, family and person supported in regards to schedule implementation and other needs for the person receiving services.

Attend and actively participate in required trainings, mandatory monthly staff meetings and supervision meetings.

Enter notes in Efforts To Outcomes (ETO) data system to ensure record of services provided.

Complete and submits timesheet and mileage reports to the Program Director by established deadline(s).

*Must have own reliable insured transportation to transport participant.
