Mee Memorial Hospital is Hiring a Clinics-CPSP Worker PT/DAYS Near King, CA

JOB SUMMARY: Conducts CPSP orientation, initial CPSP assessments, reassessments and postpartum assessments for perinatal clients. Acts as contact person and liaison between the CPSP provider and the County CPSP Coordinator. Performs other duties as assigned.

PERFORMANCE DIMENSIONS AND TASKS
Essential Function
1. Maintains the files and manuals containing the changes to application, CPSP protocols and other correspondence regarding the program.
a. Maintains the CPSP provider handbook and other staff education materials.

2. Reviews protocols
a. Annually with the program supervising physician
b. Reviews perinatal patient education material education materials and selects appropriate materials for patient population.
• Maintains an adequate supply of these materials to meet patient needs.

3. Knowledge
a. Has a working knowledge of CPSP regulations and works to ensure compliance with State guidelines.
b. Attend state-wide or county level CPSP training programs as needed and feasible either virtually or in-person.
c. Develops and maintains a knowledge of pregnancy and perinatal issues by attending appropriate perinatal workshops and in-services.
d. Ensures that all practitioners working in CPSP attend appropriate training. Keeps a record of attendance on file.
e. Participate in monthly CPSP department meetings.

4. Maintains a patient log or tacking system to ensure the patients receive appropriate services throughout pregnancy.
a. Reviews EMR documentation and patient records to ensure that patients have received appropriate and complete care according to protocols and that services are documented and billed correctly.
b. Arranges for case conferences for selected patients to review care plans and evaluate quality of care given.

5. Patient orientation, initial assessment and reassessments.
a. Ensures that patient receives orientation, initial assessments and reassessments each trimester and in the post-partum period.
b. Develops an individualized care plan which addresses the patient’s problems, needs and risk factors.
c. Updates and revises the care plan, as needed, throughout the pregnancy.
d. Ensures that interventions are provided as needed, either individually or in groups, throughout the patient’s pregnancy. Follows protocols approved by supervising physician and masters-level consultants.
e. Uses pamphlets, videos and other teaching aids, as appropriate.
f. Ensures that referrals are made for needed services not provided on site, such as; smoking cessation, drug or alcohol recovery childbirth education, etc. follows up to access compliance.
g. Ensures that the Patient is assisted with practical arrangements such as transportation, translation needs and assistance with tests, referral and special appointments.
h. Keeps track of the patient’s attendance at appointments. Identifies the reason for a missed appointment ad assist the patient with making new appointment.
i. Ensures that result of tests and referrals are given to appropriate team members and are recorded in the patient’s chart. Consults with other members of the healthcare team, as needed, to ensure the patient’s needs are communicated, addressed and care is coordinated.
j. Is available to the patient as a contact for problems and questions. Assists the patient in problem-solving.
k. Ensures that services provided are documented according to CPSP regulations. Oversees the patient’s chart for completeness of documentation of medical, health education, nutrition and psychosocial services.
l. Ensures that billing forms for services provided are filled out. Keeps track of services provided on billing summary form.

QUALIFICATIONS

Education:

Work Experience: One-year experience in perinatal care. Experience working on a multidisciplinary team.

Skill Requirements: Knowledge of basic medical and perinatal terminology procedures and symptoms. Good communication/interviewing skills. Good written communication skills.

Licensing Requirements: None

Language Requirements: Bilingual required (English/Spanish)

Physical Demands: Must be able to sit for 6 hours and stand/walk for 2 hours or more per day. Must be able to lift and push/pull from 10 to 50 pounds. Must be able to speak and hear.

Special Demands: Willing to perform repetitive tasks. Ability to work with others and to perform a variety of assigned tasks.
