Job Summary:
Supports the day-to-day coordination, administration, community engagement, and data functions for the Population Health initiatives, under the direction of the Director of Population and Community Health
Primary Duties:
1.Acts as primary agent for the coordination and implantation of the Community Health & Wellness program.
2.Assists Case Management and other hospital departments to implement outreach efforts that engage patients to adhere to care plans and/or remove barriers to care.
3.Oversees workforce staff including Community Health Advocates and Drivers.
4.Assists in the synthesis of data analysis into clear, relevant, and visually appealing materials that can be used to make strategic decisions.
5.Builds and maintains relationships with community partners.
Qualifications:
Bachelor’s degree in Public Health, Nursing, Social Work, or related field is required.
3-4 years of experience including 1 or more years of proven case management, project management and population health familiarity with the local area and/or population health workforce integration.
Licensed Clinical Social Worker is preferred.
