As the MDS Coordinator, you will serve as a key member of our interdisciplinary team, ensuring accurate and timely completion of Minimum Data Set assessments for our residents. You'll play a vital role in coordinating resident care plans, maintaining compliance with regulatory requirements, and promoting the overall well-being of our residents.

Key Responsibilities:

🔹 Coordinate and conduct comprehensive resident assessments in accordance with federal and state regulations.

🔹 Collaborate with nursing staff, therapists, and other healthcare professionals to gather necessary information for MDS assessments.

🔹 Analyze assessment data to develop individualized care plans that address residents' physical, emotional, and psychosocial needs.

🔹 Monitor and update MDS documentation to reflect changes in residents' conditions and treatment plans.

🔹 Ensure compliance with Medicare, Medicaid, and other regulatory guidelines related to MDS assessments and reimbursement.

🔹 Provide education and support to staff regarding MDS documentation requirements and best practices.

🔹 Participate in interdisciplinary team meetings to discuss resident care plans and progress.

Qualifications: 🔸 Registered Nurse (RN) or Licensed Practical Nurse (LPN) with a valid license in the state of New York.

🔸 Minimum of 2 years of experience in long-term care or a similar healthcare setting.

🔸 Certification as a Resident Assessment Coordinator (RAC-CT) preferred.

🔸 Strong understanding of MDS 3.0 guidelines and regulations.

🔸 Excellent organizational and time management skills.

🔸 Effective communication and interpersonal abilities.

🔸 Ability to work collaboratively in a team-oriented environment.
