RN Case Manager Immediate Need - Home Health Care

We are currently seeking Registered Nurses for our Home Health Programs in South Illinois region. Coverage Areas: Zip codes: Markham, Country Club Hills, Flossmoor, Hazel Crest, Homewood, Olympia Fields, Matteson, Richton Park

Requirements:

• 1 Year Experience Home Health
• BLS
• CPI (Nice to have)
• ACLS (Nice to have)
• NIHSS (Nice to have)
• EPIC

Description:

• Patients: 5-6 Patients
• Case Manager: 20-30 patients
• Shifts: 8am - 5pm
• Weekends: Monday – Friday with rotation into weekends where required
• Start Date: ASAP
• Duration: 13 weeks with high likelihood of extension
• Start Date: Mar 4, 2024 - Jun 1, 2024
• Coverage Areas: 60074,60010,60047
• Compensation: $70-$75 per hour, based on experience
• Patient Types: Patients typically have multiple comorbidities and can include (but not limited to): CHF, COPD, pneumonia, cancer, dementia, kidney disease, hypertension, fall, fracture, infections, cellulitis, post-surgical, wound care, ostomy care, labs, IV infusion, etc.
• Skills required: Wound care, including wound vacs, pleural drains, ostomy care; peripheral blood draws, central line care, IV administration through multiple devices including elastomeric and IV pumps, excellent assessment skills, experience with computer documentation
