Title : Community Health Worker (Hybrid)Location : various location available : Fort Smith, Fayetteville, Springdale, West Memphis ARSchedule : 40 Hours per week - Monday - Friday (8 : 00am - 6 : 00pm)Pay : $22 an hour 100% Mileage paid Start : March 2023 Job Summary : Administers field based Independent Assessments to beneficiaries.
Requires daily travel to and from appointments at Provider offices, beneficiary homes, and other community meeting locations within areas served by Arkansas Department of Human Services.
Communicates in a culturally sensitive manner to individuals whose support needs are being assessed within the mental health and / or developmentally disabled populations.
