GENERAL SUMMARY:
The Clinical Pharmacy Specialist-Medication Management, commonly referred to as Drug Information, is a part of the Pharmacy Shared Services team. They are responsible for ensuring the safe use of medications in the health system; managing the health system drug formulary; overseeing medication-related clinical policies and procedures, as well as ensuring the integration and consistency of local practices to such policies and procedures. They are also responsible for ensuring the cost-effectiveness and safety of medication use within the system through optimal use of data and analytics. The pharmacy specialist will also be precepting pharmacy students, residents, and fellows in the area of drug information, medication safety, and pharmacy administration. The position is based at the Centralized Pharmacy Services Center (CPSC) at the Henry Ford Health Elijah McCoy building in Detroit.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
oReviews medication safety practices, safety reporting program information, and advise on systems changes with all relevant stakeholders.
oServes as a member of the System Medication Safety Committee
oServes as a member of the Ambulatory Quality & Safety Committee
oReviews clinic medication use process for high alert medications (including look-alike/sound-alike medications) and develops policies and procedures, and/or enforces established policies and procedures, to minimize safety risks.
oReview medication use and provide recommendations, such as to support optimal medication dosage form & design, distribution, storage, and administration.
oProvide education on medication safety principles
oPromote High Reliability & Autonomous Pharmacy principles and practices.
