Key Duties

  • Direct Patient Care: Offer direct care tailored to the specific needs of individual patients, ensuring their comfort and well-being.
  • Monitoring & Assessment: Continually observe patients, recording any significant conditions and reactions to treatments, drugs, or other medical interventions.
  • Medication Administration: Administer prescribed medications to patients, ensuring proper dosages and observing for potential side effects.
  • Collaboration: Collaborate with a multidisciplinary medical team, ensuring cohesive and comprehensive care for patients. This includes regular communication with RNs, physicians, and other medical professionals.
  • Patient Education: Inform patients and their families about medical conditions, treatments, and preventive care. Address any concerns or questions they may have.
  • Documentation: Maintain thorough, accurate, and current patient records, ensuring that all relevant medical information is accessible and organized for the care team.

Qualifications

Applicants should possess a valid LPN/LVN license, demonstrate a passion for patient-centered care, and be ready to work in a dynamic, supportive environment that encourages continuous learning and professional growth.
