MECA Therapies, LLC is Hiring an Early Childhood Care Coordinator Near Las Cruces, NM

MECA Therapies, LLC if looking for a Care Coordinator to join our professional team. 

The Care Coordinator is responsible for initiating and overseeing MECA’s Early Childhood Services referrals, family introduction to MECA Therapies services, and intake process. The Care Coordinator is responsible for ensuring that a target number of monthly referrals are achieved based upon MMT goals and expectations. The Care Coordinator will be assigned specific duties by the Regional Director to support family need identification and cross referrals to other applicable MECA Therapies Early Childhood Services.

Education and Licensure A bachelor’s degree in social work; counseling; psychology, nursing; special education; early childhood education or closely related field. Individuals with a bachelor’s degree in another field can substitute two (2) years of direct experience in serving individuals with disabilities and/or families.

General Requirements:

  • Care Coordinators are required to attend Family Service Coordinator (FSC) training modules Part I and II. Care Coordinators must complete all modules within 6-months of the date of hire.
  • Complete Routine Based Interview online training within 2 weeks of hire.
  • Complete FITKIDS Database training within 2 weeks of hire
  • Proficient computer skills in Microsoft Windows
  • Proficient organizational and record keeping skills.
  • Effective oral and written communication
  • Excellent interpersonal and teaming skills
  • Ability to train clients, family members and professionals.
  • Maintain a high level of confidentiality.

ESSENTIAL DUTIES

  1. Assisting and enabling the families of children to access and receive early intervention services
  2. Entering all initial referrals into ZOHO, MAPS, and FITKIDS
  3. Processing all intakes for early intervention
  4. Contacting the family within 3 days of the date of referral to schedule intake meeting
  5. Completing Welcome to MECA Therapies and discussing MECA Therapies services with families
  6. Explaining to families the early intervention process, parent rights and safeguards
  7. Gathering information from the family regarding their concerns, priorities and resources using the Routine Based Interview (RBI)
  8. Completing initial identified Care Coordinator Intake Forms with families
  9. Connecting the family with resources and supports
  10. Responsible for notifying family of next steps and informing MECA Therapies Team (HV, FIT, etc.) to contact the family to continue the process with a warm handoff to the appropriate program(s)
  11. Coordinating with Intake teams for scheduling
  12. Verifying Medicaid
  13. Coordinating with medical and health providers
  14. Requesting all medical records
  15. Following the Close/Exit timeline process for families that are difficult to reach.
  16. Participating in screenings as assigned
  17. Marketing all MECA Therapies services to all referral providers
  18. Following up with all referral sources and extending a Thank You for referrals received
  19. Providing services to all regions served as applicable.
  20. Providing services via telehealth as assigned
  21. Other duties as assigned.

CASELOAD

The Care Coordinator will work with the Regional Leadership Teams to onboard children and families to ensure our staffing and caseload expectations are met on monthly basis.

PRODUCTIVITY

The Care Coordinator will work to meet goals and outcomes for their respective regions as it relates to Key Performance Indictors set by the Executive Team. The Care Coordinator will achieve the expected productivity set by MECA Management by:

  1. Meet and exceed KPI for programs within each region.
  2. Create more efficient referral- to service model.
  3. Together with the Regional Leadership Team expand referral base.

DOCUMENTATION

The Care Coordinator will complete necessary data collection and reporting tools as mandated by MECA executive management, programs, and primary contracting agencies including NM Department of Health, Early Childhood Education and Care Department (ECECD), and Local Education Agencies and will ensure that the information is accurate and filed in a timely manner by adhering to the following:

  1. Creating and maintaining the initial confidential records for each person which

includes the following:

  1. Family Service Coordination Log
  2. Signed consents and acknowledgments.
  3. Release and obtain of information.
  4. Welcome Information
  5. Documenting all time spent with the family and work done on behalf of the family
  6. Completing and turning in all monthly documentation and billing submissions in accordance with program specific guidelines
  7. Documenting all steps taken within ZOHO, MAPS, and FITKIDS
  8. Maintaining record of referral and status
  9. Maintaining record of cross referrals

STAFF DEVELOPMENT AND TRAINING

The Care Coordinator will comply with all mandatory trainings identified by the Regional Director and MECA Management Team by adhering to the following:

  1. Consulting, mentoring, and providing in-house trainings to other staff members as appropriate.
  2. Attending mandatory staff meetings and trainings

SUPERVISION

The Care Coordinator will receive general supervision from their Regional Director and MECA Management Team as detailed in MECA Therapies policies and procedures. Care Coordinator will work in conjunction with the Regional Director and as a member of the Regional Leadership Team.

QUALITY ASSURANCE

  1. Auditing the files in the initial process for accuracy and completeness in a timely manner and prior to providing the file to the intake coordinator
  2. Ensuring that all Care Coordination (service coordination) services and related activities are ethical and in compliance with MECA Therapies policies and procedures.
