Meadowbrook manor in Naperville is now hiring a Social Services Director to join our team.

The Social Services Director requires appropriately transitioning the resident and family through the care process, which includes the ability to assess cognitive capacity and mood assessments utilizing standard tools. Implementation of healthcare directives and ability to have discussions and knowledge of end-of-life care and palliative/hospice. Understanding of the fundamentals of discharge planning and be able to work collaboratively with health care teams which includes nursing and physician.

Requirements

• Minimum of BS in Social Work.
• Prior experience in Social Service work in a LTC setting preferred.
• Manage QIDP's/Case managers in ensuring Care Plans are updated and relayed to Direct Care staff to help residents meet goals.
• Ability to motivate, encourage residents; knowledge of how to adapt life skills to the cognitive and functional ability of each resident and attempt to meet goals.
• Ability to handle multiple priorities.
• Possess written and verbal skills for effective communication.
• Competent in organizational & time management skills.
• Demonstrates good judgment, problem solving and decision-making skills.
• Knowledge of state and federal resident long-term care regulations in a LTC setting.
• Excellent communication, leadership, and mentoring skills
• Must demonstrate strong conflict resolution.
• Proficiency understanding Medicare/Insurance/Medicaid
