We are currently seeking a dedicated Licensed Vocational Nurse (LVN) to join our team. As an LVN, you will be responsible for providing direct patient care and assisting in the implementation of various healthcare programs. Responsibilities

  • Administer medications and treatments to patients as prescribed by physicians or registered nurses
  • Monitor vital signs and report any abnormalities to the appropriate healthcare professional
  • Assist patients with activities of daily living, such as bathing, dressing, and grooming
  • Document patient information, including medical history, symptoms, and progress notes
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans
  • Conduct health screenings and assist in the coordination of healthcare services
  • Educate patients and their families on preventive care and disease management

Requirements

  • Must have a valid LVN license and be in good standing with the state of CA

Our subacute healthcare facility is dedicated to providing specialized care for patients who require ongoing medical support following a hospital stay. As an LVN Subacute, you will be responsible for monitoring and administering medications, assisting with daily living activities, and collaborating with a multidisciplinary team to develop and implement comprehensive care plans.
