Job Description

Definition: The purpose of this position is to provide case management for the Juvenile Substance Abuse Crisis Center. This is accomplished by providing intake services for clients; monitoring clients; completing referrals; and tracking client progress throughout the program. Other duties include performing community outreach, group facilitation and prevention efforts.


Job Responsibilities

Essential Job Functions: Note: This information is intended to be descriptive of the key responsibilities of the position. Completes client intake by receiving referral; contacting family; explaining services; setting up intake interviews; coordinates and monitors services, including comprehensive tracking of client activities in relation to care plan.
Monitors client progress by setting up outpatient and inpatient services; making follow up checks at home, school, and within community; providing drug testing as needed; documenting all client activity; attending school meetings for client; completing 6 month and twelve month client follow ups; and providing educational materials regarding alcohol and drug use.
Completes referrals and discharges by writing referrals to appropriate agencies for client services; completing referral packets; advocating for admission; making reminder calls to families; and assisting with job search and applications.
Actively seeks collaboration opportunities with community partners in order to perform community outreach and prevention efforts by attending and participating in community agency meetings, events, health fairs and presentations; providing materials; presenting relevant information; setting up events; facilitating evidence-based curriculum to clients and youth in the county; works closely with the schools to schedule evidence-based prevention activities including groups and presentations to students.

Knowledge, Skills and Abilities

Knowledge, Skills, and Abilities Required:
  • Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community.
  • Knowledge of community health care and vocational services.
  • Knowledge of youth focused resources within McKinley County.
  • Knowledge of up-to-date alcohol and drug use trends among youth.
  • Knowledge of case management principles, objectives, standards and methods.
  • Ability to use motivational interviewing techniques while working with and interviewing clients.
  • Skill in Records maintenance and time management.
  • Ability to gather data, compile information, and prepare reports.

Minimum Qualifications:
  • High School Diploma or GED equivalent;
  • A combination of two (2) years of experience or education in the areas of social work, criminal justice, law enforcement, or related field.
Preferred Qualifications:
  • Bachelor’s Degree in social work, criminal justice, law enforcement administration or related field.
  • At least 3 years of experience directly related to the duties and responsibilities specified
  • Bilingual (Navajo and/or Spanish)
  • Experience working in a county/local government environment.


Additional Information

Other Requirements: The incumbent of this position must comply with established safety guidelines of the County.
Supervisory Controls: The incumbent performs under the administrative supervision of the Community Services Director.
Working Conditions: Work is performed in an office setting. Work schedule may include irregular hours. Working outside, doing home site visits, being exposed to weather and other environmental and/or social conditions.
Certificates and Licenses: Must possess a valid Driver’s License at time of employment and be insurable for liability purposes; and the ability to possess a New Mexico Driver’s License within six (6) months of hire date.

Conditions of Employment: May be required to pass a post offer-of-employment physical examination and a medical history check.
