We are looking for an experienced MDS Coordinator to join our team. The ideal candidate will have experience working with geriatrics in a long-term care setting, preferably in a nursing home. The MDS Coordinator will be responsible for coordinating and completing the Minimum Data Set (MDS) assessments, and providing guidance and support to the nursing staff. This is an excellent opportunity for a motivated individual to work with a team of dedicated professionals in a supportive environment.

Responsibilities:

• Develop and implement a plan of care for residents in long-term care facilities.
• Assess the needs of residents and develop individualized care plans.
• Monitor the progress of residents and make necessary changes to their care plans.
• Provide education and support to families of residents.
• Coordinate with other healthcare professionals to ensure the best possible care for residents.
• Ensure compliance with all state and federal regulations regarding long-term care facilities.

Job Type: Full-time

Pay: $27.00 - $32.00 per hour
