Job Description:
Oversees the needs of individuals dealing with homeless and housing insecurities, assists with linking them to necessary services and resources related to their housing needs, and provides the necessary monitoring, follow-up, and coordination of services. The Rapid Re-Housing Coordinator works for the Arc of Safety, for which Mazzoni Center is the fiscal sponsor.
Communication Skills
Good oral and written communication skills are required for this position. The person in this position must be able to communicate effectively with the clients, staff, and others; and contribute to a positive environment.
Key Responsibilities
Client Care
● Provide a minimum of 19 hours per month of direct client services.
● Assess the housing, employment, social services, medical, and other program-related needs of clients and develop service care plan
● Maintain minimum contact with clients based on program, client need, and service care plan
● Identify appropriate resources to address client needs and assist client with linkage to those resources
● Coordinate and monitor ongoing services for client
● Assist client with identifying, building, and maintaining external support systems
● Initiate the intake process for all the clients that enter the program
● Perform face to face housing and social assessment with all new applicants
● Address complex issues faced by families experiencing homelessness (including, but limited to unstable housing history, trauma, etc.)
● This position is responsible for providing trauma-informed, person-centered
housing and housing focused case management services, collaborating with various participant stakeholders, and system collaboration with other community service providers
Documentation
● Maintain accurate written documentation of all client contacts, service units/visit encounters and case notes; such information is expected to be input within 24 hours of contact with client.
● Maintain accurate and orderly client files with hard copy of client-related forms, applications, correspondence, etc.
● Protect client information and records at all times in compliance with Pennsylvania Act 148, the Health Insurance Portability and Accountability Act (“HIPAA”), and organization’s Confidentiality Policy.
Other Qualifications and Skills.
● Bachelor's Degree in Social Services (Psychology, Social Work, Nursing, Public Health) or equivalent lived experience
● Knowledge of community resources
● LGBTQ cultural competency
● Competency in working with diverse populations
● Computer literate
● Bi-lingual preferred
● Prior direct service experience with people experiencing homelessness and/or those with substance abuse or behavioral/mental health issues
● Familiarity with social, health and benefit resources
● Two years of Case Management or Social Work experience
● Knowledge of local affordable housing market Clearances
● Participate in weekly supervision with the Ark of Safety Executive Director
● Participate in organization meetings, staff in-services, group supervision, and external training as required
● Perform other program related duties as assigned by the Ark of Safety Executive Director.
Clearances: Background checks are required, including Criminal Background Clearance, Child Abuse Clearance, and FBI Clearance.
