Maya Home Health Care is seeking qualified Physical Therapists and Physical Therapist Assistants to provide one-on-one skilled physical therapy to patients living in their own homes. PT develops and implements a plan of treatment in conjunction with the physician and the patient. PT collaborates with other interdisciplinary members in case managing the care and treatment necessary to improve the patient's health condition.

Requirements:

• Graduate of an accredited school of physical therapy
• Current license to practice as a physical therapist in MA.
• Drivers license and proof of car insurance
• Valid CPR license
• Ability and willingness to work in a team environment.
• Ability to adjust schedule availability to meet the needs of the patients.
• Ability to travel between patient assignments.

This position is Monday through Friday. Competitive wages plus mileage.

Job Types: Part-time, Per diem

Pay: $45.00 - $50.00 per visit for PTA

$75-150 per visit for PT
