Shifts available: Weekends - night shift

Pay rate: $34.00 - $36.00/hour

LPN Job Responsibilities May Include:

• Participate in the nursing plan and in implementing the nursing plan of care on the assigned shift and unit in accordance with current applicable federal, state, and local standards, guidelines, and regulations within the individual's scope of practice as applicable and may be delegated by the DON or Administrator to assure that the highest degree of quality resident care can be maintained.
• Conducts daily resident rounds to assess physical and mental health status.
• Implements any required nursing interventions and documents appropriately.
• Coordinates admission, discharge, and transfer of residents.
• Assure that stock level of medications, medical supplies, equipment, etc are maintained at adequate levels. Requisition equipment and supply needs as required.
• Informs the charge nurse or others (as appropriate) about the status of residents and related matters.
• Participate in resident and family education relating to resident care.

Job Requirements include (but not limited to):

• Current LPN license in good standing
• American Heart BLS
• Preferred experience in rehab and skilled nursing
